# Human review

## Human review is required

Axiom Cortex provides structured decision support. A qualified human reviewer remains responsible for understanding the role, inspecting the evidence, correcting invalid findings, applying company policy and law, and making the final decision.

## Reviewer checklist

Before using a report, the reviewer should verify:

1. The role description and must haves are current, job related, and approved.
2. The questions actually test the configured criteria.
3. The ideal answer criteria describe observable job evidence and do not require one exact wording or background.
4. The transcript is complete enough, accurately attributed, and usable.
5. The candidate received a fair opportunity to address each material criterion.
6. Evidence references support the finding without removing context.
7. Personal ownership is not inferred from a team outcome.
8. Missing evidence is not treated as an automatic failure.
9. No protected trait, accent, disability, appearance, emotion, or clinical inference affected the review.
10. Any accommodation, language, or interview design issue was handled appropriately.
11. Material contradictions were inspected in the source transcript.
12. The final decision follows applicable law and the company's approved process.

## When to request more evidence

The reviewer should request more evidence when a core criterion was not tested, the transcript is incomplete, attribution is unclear, the question was interrupted, or the answer supports only a narrow part of a material requirement.

## When to exclude a finding

A finding should be excluded when it relies on an irrelevant criterion, an inaccurate transcript, mistaken speaker attribution, a protected characteristic, an unsupported interpretation, or an invalid interview opportunity.

## Corrections and accountability

Corrections should preserve the original source reference, the reason for the change, the reviewer identity or authorized role, and the updated review state. This creates an inspectable record without exposing private internal reasoning.

## Decision boundary

The product report is one input to a broader process. References, work samples, technical exercises, lawful background information, business constraints, and additional interviews may provide separate evidence. Those sources should not be silently represented as Axiom Cortex interview evidence.
